| Erik
van Dijk, P.T.
Erik van Dijk is the founder and owner of Physical
Therapy Associates. In 1978 Erik received his degree
in Physical Therapy from SUPA, Academy for Physical
Therapy, at Utrecht, The Netherlands.
Since then he has worked in a variety of healthcare
settings such as acute care, rehabilitation, home care
and outpatient settings as well as education prior to
establishing Physical Therapy Associates.
The extensive experience and training were the foundation
for the start of Physical Therapy Associates, providing
hands-on care on a one-on-one basis at the patients
convenience.
Many physicians and patients alike have come to depend
on the skill and experience that Erik offers in his
treatments.
He is a member of the American Physical Therapy Association,
the Dutch Physical Therapy Association and the Anglo
Dutch Institute for Oriental Medicine.

| Atheena
M. Maragos-van Dijk, P.T.A.
Atheena Maragos-van Dijk graduated with Distinction
from Kent State University in 1990 and earned her Associate
Degree in Applied Science of Physical Therapy Assistance.
She has been instrumental in the development and growth
of Physical Therapy Associates. Her skill and compassion
have provided many patients, young and old, with much
needed comfort and hope for the future.
Her continuing education credits include coursework
in general orthopedics, shoulder injuries, total joint
replacement of the knee, hip and shoulder, developmental
delay in pediatric patients, legal and ethical considerations
in rehabilitation and strain/counterstrain treatment
technique.
Atheena has provided assistance to the Jefferson County
Health Department "health walks" and presented
a lecture in cooperation with Dr. John Seifert at Sonshine
Youth Athletic Center involving the Benefits of Exercise.
She is a member of the Ohio Chapter of the American
Physical Therapy Association and the Orthopedic Section
of the A.P.T.A.

| Dana
Brown, Office Manager, Wintersville Office
Dana
has been employed by Physical Therapy Associates since
1995. She carries out many clerical duties involved
in our operation: taking referrals and patient intakes,
checking insurance benefits, helping patients file claims,
and corresponding with doctorsoffices, workers
compensation and insurance companies.
Anyone
who has had experience in working with Dana knows that
she will not rest until things get done!

| Jan
Maragos, Billing Manager
Jan
has been with Physical Therapy Associates since 1998
and is in charge of all aspects of billing. She files
claims for treatments provided. Jan graduated from The
Ohio State University with a Bachelors of Science in
Education in 1982 and a Masters of Arts in 1994. She
previously was employed as a teacher in the Hilliard
Public School District. Jan is available to take all
your billing questions and inquiries.

| Amy
Westlake, P.T.A.
Amy
joined Physical Therapy Associates in May of 2001 and
brings with her a variety of experience through her
previous positions in outpatient care, skilled nursing
and pediatrics. Amy graduated from Kent State University
in 1995 with an Associate Degree of Applied Science
in Physical Therapist Assistant Technology.
Amy
adds years of valuable experience and believes in a
team approach to provide holistic rehabilitation.

| Connie
Shea, R.N.
Connie
graduated in 1983 from Mount Carmel School of Nursing,
Columbus, Ohio and has extensive experience in Diabetes
and Renal care, Critical care and Home care. Connie
joined Physical Therapy Associates in September 2002
and is involved as a liason between Physical Therapy
Associates and physician offices, workers compensation
and managed care organizations.

Margeaux L. Mallas, MPT
Margeaux joined Physical Therapy Associates in August of 2004 after graduating from Wheeling Jesuit University with a Masters Degree of Physical Therapy, a Bachelor of Arts in Psychology and is currently enrolled in the graduate program which will earn her a Doctorate Degree of Physical Therapy in August 2005. Margeaux is a lifetime resident of Jefferson County, having lived most of her life in Mingo Junction.
Margeaux’s volunteer experiences involves a long list of activities which revolve mostly around health, sports and fitness in various settings. We are excited to have Margeaux on staff as she will be the first doctor of physical therapy in this area and are convinced that you will appreciate her skill and knowledge as much as we do at Physical Therapy Associates.

| Lori Elliott, P.T.A, C.P.I.
Lori joined Physical Therapy Associates in June of 2004. Lori graduated from Kent State University in 1995 with an Associate Degree in Physical Therapy Assisting Technology.
She recently completed her training as a Certified Pilates Instructor. Previously, Lori worked in various settings of physical therapy in Youngstown and Warren. Lori resides in Wintersville.

Lisa joined Physical Therapy Associates in January of 2004 and is responsible for all referrals, authorizations and other necessary documentation as required by the insurance companies of our clients. You may
